A robotic system at Stanford Medical Center was used to perform a laparoscopic gastric bypass surgery successfully with a theoretically similar rate of complications to that seen in standard operations. However, as there were only 10 people in the experimental group (and another 10 in the control group), this is not a statistically significant sample.

If this surgical procedure is as successful in large-scale studies, it may lead the way for the use of robotic surgery in even more delicate procedures, such as heart surgery. Note that this is not a fully automated system, as a human doctor controls the operation via remote control. Laparoscopic gastric bypass surgery is a treatment for obesity.

There were concerns that doctors, in the future, might only be trained in the remote control procedure. Ronald G. Latimer, M.D., of Santa Barbara, CA, warned “The fact that surgeons may have to open the patient or might actually need to revert to standard laparoscopic techniques demands that this basic training be a requirement before a robot is purchased. Robots do malfunction, so a backup system is imperative. We should not be seduced to buy this instrument to train surgeons if they are not able to do the primary operations themselves.”

There are precedents for just such a problem occurring. A previous “new technology”, the electrocardiogram (ECG), has lead to a lack of basic education on the older technology, the stethoscope. As a result, many heart conditions now go undiagnosed, especially in children and others who rarely undergo an ECG procedure.

Australian health workers to close intensive care units in Victoria next week

Members of Australia’s Health Services Union (HSU) will go on strike in Victoria next week in a dispute over stalled wage and career structure negotiations. Over 5000 physiotherapists, speech pathologists and radiation therapists will walk off the job next week, effectively closing the state’s 68 largest health services.

The strike will force the closure of intensive care units and emergency departments across the state.

It is feared the strike could continue into Easter.

National secretary of the HSU, Kathy Jackson said admissions would be crippled, while intensive care patients would have to be evacuated to New South Wales, Tasmania and South Australia as hospitals will not be able to perform tests or administer treatment.

“When an ambulance shows up you can’t admit a patient without an X-ray being available, you can’t intubate them and you can’t operate on them,” she said.

“If something goes wrong in an ICU you need to be able to X-ray, use nuclear medicine or any diagnostic procedure,” said Ms Jackson.

Ms Jackson said the HSU offered arbitration last year, but the state government refused. “They’re not interested in settling disputes, they hope that we are just going to go away.”

“We’re not going away, we’ve gone back and balloted the whole public health workforce in Victoria, those ballots were successful, 97 percent approval rating,” she said.

The HSU is urging the government to commence serious negotiations to resolve the dispute before industrial action commenced.

The government has offered the union a 3.25 per cent pay increase, in line with other public sector workers but the union has demanded more, but stopped short of specifying a figure.

Victorian Premier John Brumby said the claim would be settled according to the government’s wages policy. “The Government is always willing and wanting to sit down and negotiate with the relevant organisations . . . we have a wages policy based around an increase of 3.25 per cent and, above that, productivity offset,” he told parliament.

The union claims it is also arguing against a lack of career structure, which has caused many professionals to leave the health service. Ms Jackson said wages and career structures in Victoria were behind other states.

Victorian Opposition Leader Ted Baillieu said he was not in support of the proposed strike and called on the government to meet with unions. “There could not be a more serious threat to our health system than has been announced today.”

“We now have to do whatever is possible to stop this strike from proceeding,” he said.

The opposition leader will meet with the union at 11:30 AM today.

Victorian Hospitals Industry Association industrial relations services manager Simon Chant said hospitals were looking at the possible impact and warned that patients may have to be evacuated interstate if the strike goes ahead.

Liu spoke with Technological University Dublin (TUD) senior lecturer at the School of Surveying & Construction Management, Dr Lorcan Sirr on Friday regarding the supply of housing in the Republic of Ireland and relevant parallels across the rest of Europe, as well as recent developments by the government and private sector that are causing a rise in rents and home prices in the Irish real estate market.

U.S. Supreme Court upholds health care mandate

In a decision today, the U.S. Supreme Court has upheld the controversial healthcare law, the Patient Protection and Affordable Care Act, passed in 2010. The Supreme Court also upheld the individual mandate provision of the law, which would require most U.S. citizens to obtain health insurance by 2014, or pay a monetary penalty.

The Supreme Court ruled on the law 5–4. Chief Justice John Roberts authored the majority opinion. Justices Stephen Breyer, Ruth Bader Ginsburg, Elena Kagan and Sonia Sotomayor jointed Roberts in the majority, while Samuel Alito, Anthony Kennedy, Antonin Scalia and Clarence Thomas were in the minority in supporting the repeal of the law.

The Court did, however, strike down a provision of the law which would have expanded Medicaid to make coverage available to anyone with an income less than 138% of the federal poverty line.

President Barack Obama made a public statement from the White House saying that the Supreme Court’s upholding of the law, “reaffirmed a fundamental principle that here in America — in the wealthiest nation on Earth – no illness or accident should lead to any family’s financial ruin.” Obama further added, “Whatever the politics, today’s decision was a victory for people all over this country whose lives will be more secure because of this law and the Supreme Court’s decision to uphold it.” Obama closed his statement by saying, “The highest Court in the land has now spoken. We will continue to implement this law. And we’ll work together to improve on it where we can. But what we won’t do — what the country can’t afford to do — is refight the political battles of two years ago, or go back to the way things were.” Adding, “With today’s announcement, it’s time for us to move forward — to implement and, where necessary, improve on this law.”

Republican presidential candidate Mitt Romney also gave a statement from Washington, D.C. saying, “What the Court did not do on its last day in session, I will do on my first day if elected President of the United States. And that is I will act to repeal Obamacare.” Romney clarified further by saying, “Let’s make clear that we understand what the Court did and did not do. What the Court did today was say that Obamacare does not violate the Constitution. What they did not do was say that Obamacare is good law or that it’s good policy.” Further adding, “Obamacare was bad policy yesterday. It’s bad policy today. Obamacare was bad law yesterday. It’s bad law today.” Romney closed by saying, “Our mission is clear: If we want to get rid of Obamacare, we’re going to have to replace President Obama.”

Eric Cantor, Republican leader in the U.S. House of Representatives, announced shortly after the ruling that the House would vote on repealing the law on July 11, following the July 4 holiday recess. Cantor said, “The Supreme Court’s decision to uphold Obamacare is a crushing blow to patients throughout the country. Obamacare has failed to keep the President’s basic promise of allowing those who like their health care to keep it, while increasing costs and reducing access to quality care for patients.”

In the ruling of National Federation of Independent Business v. Sebelius, John Roberts wrote, “Simply put, Congress may tax and spend. The federal government may enact a tax on an activity that it cannot authorize, forbid or otherwise control.” Meanwhile, Justice Anthony Kennedy authored the dissenting opinion, saying the entire law should have been repealed.

The ruling on the law comes after 26 states challenged the law in oral arguments in front of the Supreme Court in March.

Creekstone Farms sues USDA for refusing to allow mad cow testing

Creekstone Farms Premium Beef, an organic meatpacking company based in Arkansas City, Kansas, filed a lawsuit against the U.S. Department of Agriculture (USDA) for what the company claims is threats by the USDA that it would face prosecution if it proceeds with plans to test nearly 100% of its beef for Bovine spongiform encephalopathy, also known as mad cow disease or BSE.

The USDA position is that allowing any meatpacking company to test every cow would undermine the agency’s official position, a position that holds to random testing as a scientifically adequate measure to assure safety. It also claims that 100% testing does not ensure food safety because the disease is difficult to detect in younger animals.

The USDA currently tests about 1% of cattle slaughtered in the U.S. The USDA’s objection is believed to be the result of pressure from larger meatpacking operations. In 2004, the president of the National Cattlemen’s Beef Association told the Washington Post that “If testing is allowed at Creekstone, we think it would become the international standard and the domestic standard, too.” Creekstone Farms says tests cost about $20 per animal, increasing the cost of beef by about 10 cents per pound.

Every year, thousands of Americans suffer from diseases or injuries that require them to use a wheelchair. What type of impact does this have on society? If you are the loved one of one of those who must use a wheelchair, you may need to perform wheelchair accessible construction on the home to make it easier for them to move around and reach things. If you are the owner of a business, federal law may require that you make your facilities handicap accessible. The Americans with Disabilities Act (ADA) requires that public accommodations meet certain standards as to not discriminate against the handicapped, including those in wheelchairs.

What type of wheelchair accessible construction are you looking at? Homes as well as businesses should have wheelchair ramps, grab bars and handrails, and commercial washroom accessories. If you are helping a person with a wheelchair with their home, you will want something for them to sit on while showering, so a folding shower or tub seat will be necessary.

As far as wheelchair ramps, there are many types from which to choose. There are modular wheelchair ramps (strong, yet portable and affordable), solid wheelchair ramps (one piece more desirable if a permanent ramp is needed), folding wheelchair ramp (durable and lightweight for travel), and threshold wheelchair ramps (specifically designed for doorways). You will want to ensure the safety of any ramp you purchase, so as the dealer if it is ADA compliant. A slip resistance high traction surface is very important to have. Grab bars and handrails are great accessories to have for those in wheelchairs, especially in the restrooms. These bars and handrails provide stability and extra support for those who need it. Handrails can also be used beside a ramp and is especially helpful if the ramp is very long. It helps the individual keep their balance and provides safety. Grab bars should be mounted securely. This involves screwing the hardware directly into wall studs or wood blocking. The ADA says that handrails must be securely anchored, mounted between 34 and 38 inches high, projected from the wall 1 1/2 inches, and the ends must be rounded off for safety.

Shower and tub seating is also a very important part of making a home ready for someone using a wheelchair. There are different types of seating including L-shaped shower seats and folding tub seats. It will probably be more convenient to have the seats mounted to the wall, rather than have seats that the individual has to place in and out of the tub.

The most important thing when working with wheelchair accessible construction is safety and security. Please ensure that all additions made are safe for the person to use and comply with ADA standards.
